Appointment Request

Our call center staff will assist you in getting the appointment process started. Many of our cancer clinics will ask for information pertaining to your case. Here is a list of general items that we usually require be sent to us before scheduling an appointment:
- Current Medical Records, done outside of U of M including medical history from the time of diagnosis, chemotherapy records
- Operative reports
- Radiology films, including CT scans (CAT Scans, Bone Scans), MRI (Magnetic Resonance Imaging. Please send most recent scans within 6 months pertaining to the sarcoma only. If you had surgery, please send pre-operative films of tumor and post-operative films.
- All original Pathology slides and reports
The clinic coordinator will give you more specific guidelines of what to send when you call.
Depending on your medical insurance, you may also need to get pre-authorization.
Alternative to in-person appointments
Our commitment to you is to provide the care you need, when you need it, as safely and conveniently as possible. For some of our patients, in-person appointments may not be convenient or safe. Michigan Medicine also offers Virtual Care Video Visits. In order to take advantage of this option, you must have a MyUofMHealth.org account. To request a Clinic-Scheduled Video Visit, call the clinic you are interested in being seen by and ask about scheduling a Video Visit. If the type of evaluation or care you need can be done through a Video Visit, we will help you schedule a Video Visit appointment. Get more information here:

Medical insurance coverage varies greatly and can be confusing. You need to confirm the details of your coverage before making a first-time appointment. Here are the steps to follow to verify your insurance benefits:
- Contact your Primary Care Physician's office (note: the doctor who referred you to the cancer center may not be your Primary Care Physician). If you do not know the name of your Primary Care Physician, call the phone number on your insurance card for assistance.
- Your insurance provider may require your Primary Care Physician to complete a referral form prior to your visit.
- Sometimes a visit to a specialist is not a covered benefit. To help you navigate through the paperwork, the U-M Rogel Cancer Center has a Financial Services Department dedicated to simplifying the process.
- If at any time you have questions, you may call our Financial Services Office at 734-615-0396 and leave a message. A member of our Business Services Team will return your call.
